Prior X-Rays, MRIs and CT Scans
If you have had prior X-rays, MRIs or CT scans performed, we request that you bring the actual images (usually provided on a CD) AND the report to your office visit. Repeat testing may otherwise be required.
Appointment Cancellation & Re-scheduling – 24-Hour Notice
We understand that you may need to cancel or reschedule. Our friendly staff is eager to help and would appreciate 24-hour notice when you are unable to keep an appointment.
Please Bring Your Insurance Card
Insurance companies require that patients bring their insurance cards with them to all visits to verify identify. Our office needs this information to file claims with insurance companies.
Tracking Changes in Insurance Coverage
It is important that you track if your insurance coverage has changed or what restrictions may apply. We encourage you to contact your insurance company with questions regarding coverage and participating providers.
Primary Care Physician Referrals
Some insurance policies require that patients obtain a referral from their primary physician. If this is the situation with for your insurance, please contact your Primary Care Physician to obtain the referral and have it submitted to our office.
If there is no valid referral on file upon check-in, please note that you will be financially responsible for services rendered in the event that your insurance payment refuses your claim. For this reason, we recommend that prior to your scheduled visit, you call our office so that we may verify having received a referral from your primary care physician.
Payments
Co-payments are expected at the time of your visit. Payments are accepted in the form of cash or credit card. Please discuss payments options with staff if you are seeking treatment without insurance.
Patient Forms – Disability & FMLA
We are more than happy to fill-out and complete your required forms for disability, insurance, and other such documents. There is a fee, payed in full, incurred PRIOR to the completion of these forms. Please allow up to 2 (two) weeks for the completion of forms. Standard Work/School notes are provided without charge.
Workers Compensation
If you have been injured on the job, Dr. Arora is here to help. Dr. Arora accepts Workers Compensation insurances when you are being treated for an on-the-job injury.
When you are making an appointment please inform our staff that you have reported a work compensation injury to your employer.
Workers Compensation – Information We Will Need from You
In order to service you quickly and efficiently, we will need five key pieces of information from you. Please provide the following details when you call for an appointment. This information is necessary so that we can obtain prior authorization on your behalf (if it is not already in place):
1.Name of Workers Compensation Insurance Carrier
2.Name of your Claims Adjuster, phone # and fax #r
3.Claim Number
4.Date of Injury
5.Name, address and phone number of employer
Additionally, please note that it is your responsibility to have each visit approved by your adjuster per Workers Compensation requirements.
